How the debt gets counted
The difference between a physician programme and a conventional one is largely how it treats a balance that is deferred, in forbearance, or on an income-driven plan during training.
Conventional underwriting generally has to use a payment figure even when nothing is currently being paid. Physician programmes are built around the reality that a resident's current payment bears no relation to an attending's capacity.

★★ What a pending MSLRP award does, honestly
This is where we see the most wishful thinking, so here it is plainly.
It does not reduce your balance at underwriting. MSLRP agreements for the 2026 cycle begin 1 October 2026. If you close in spring on a July start, the award is a future event and your balance at underwriting is your balance.
It is a stream, not a cheque. $300,000 is the maximum over up to 10 years, reached through consecutive two-year agreements that you compete for each time. MDHHS states participation is "contingent upon the availability of program funds at the time of contracting."
What it does do is change the trajectory. A physician with a credible path to $300,000 of repayment has a different ten-year picture than the same balance with no such path, and that is worth discussing when deciding how much house to buy, even though it is not a line in the ratio today. ★ One thing to be careful about
Do not let an expected award justify a payment you cannot carry without it. The award depends on being selected, on your employer contributing its 20%, on staying with that employer for the full two years, and on the programme being funded.
Any of those can fail, and the mortgage does not care. Buy what the salary supports; treat the award as upside.
